What Stage IV Means
Any T, any N, M1 — Distant metastases present (liver, peritoneum, lungs, distant lymph nodes). Metastatic disease.
Treatment Approach for Stage IV
Palliative systemic chemotherapy: FOLFIRINOX for fit patients (ECOG PS 0-1), gemcitabine/nab-paclitaxel for others. Olaparib for BRCA-mutant metastatic disease (maintenance after platinum-based chemotherapy). Pembrolizumab for MSI-H or dMMR tumors. Best supportive care as disease progresses.
Survival Rates and Prognosis
The 5-year survival rate for pancreatic cancer at Stage IV is approximately 5-year survival: <5% (median OS ~6-9 months with FOLFIRINOX, ~8-10 months with gemcitabine/nab-paclitaxel). Survival rates are statistical averages drawn from large patient populations and may not reflect your individual outcome. Factors that influence your specific prognosis include your age, overall health, tumour biology, and how well the cancer responds to treatment.
Important: These numbers are drawn from historical data. Newer treatments available today may improve outcomes beyond what published statistics show. Discuss your individual prognosis with your oncologist.
How Other Stages Compare
| Stage | 5-Year Survival | Treatment |
|---|---|---|
| Stage IA | 5-year survival: 14-20% in surgical series | Surgical resection (Whipple or distal pancreatectomy) followed by adjuvant… |
| Stage IB | 5-year survival: 12-16% in surgical series | Surgical resection (Whipple or distal pancreatectomy) followed by adjuvant… |
| Stage IIA | 5-year survival: 7-12% in surgical series | Neoadjuvant FOLFIRINOX preferred, then surgical resection if response adequate…. |
| Stage IIB | 5-year survival: 5-9% in surgical series | Neoadjuvant FOLFIRINOX for 2 months, restaging, then surgical resection if… |
| Stage III | 5-year survival: 2-5% (median OS ~12-15 months with chemotherapy) | Neoadjuvant FOLFIRINOX for 4-6 months with reassessment for resectability. If… |
